Anti-Sweat Botulinum Toxin

Anti-sweat botulinum toxin treats excessive sweating of the underarms, hands or feet. We use Nabota botulinum toxin. It is a safe and effective method that gives longer-lasting results than topical treatments, without the need for invasive surgery. Of all the non-invasive treatments for sweating, it is the longest lasting.
When most people hear "Botox", they think of wrinkles. But this is one of the most useful uses of botulinum toxin, and very few people know about it. What it is and how it works
The eccrine sweat glands in the palms, underarms and feet are switched on by a nerve signal carried by a chemical messenger called acetylcholine. Botulinum toxin temporarily blocks that signal, so the glands in the treated area simply produce far less sweat. This mechanism is well documented in dermatology research (British Journal of Dermatology / PMC).
The botulinum toxin is injected directly into the skin where the excessive sweating occurs. The U.S. FDA approved botulinum toxin for excessive underarm sweating (primary axillary hyperhidrosis) in 2004. It is also used to reduce sweating in other areas, such as the hands and feet.
Treatment does not cure hyperhidrosis permanently. The effect lasts many months and the treatment can be repeated.

Who it is for
Hyperhidrosis is excessive sweating of the hands, underarms or feet that has nothing to do with heat or exercise. It is more common than people realize, affecting an estimated 1 to 4% of the population, and probably more, since many people never seek help (prevalence and quality-of-life data, PMC).
It is not a trivial problem. Sweaty palms can quietly erode confidence: the dread of shaking hands, of holding a pen in a meeting, of a presentation where your hands give you away. Many people with underarm sweating avoid tight-fitting or colored clothes. Research consistently shows that hyperhidrosis affects social and working life as much as some serious chronic illnesses do.
This treatment is for anyone whose sweating of the underarms, hands or feet affects their comfort, confidence or daily life.
Who should not have it
People with a skin infection in the treatment area, or a systemic infection. We ask you to return once the infection has cleared.

The treatment
The treatment takes 45 to 60 minutes.
Interview and discussion. Your doctor assesses your sweating problem and its causes, checks for contraindications and plans your treatment.
Mapping the sweating area. For the underarms, we usually define the area to treat by the hair-bearing skin, which is marked with a pen. Where needed, a Minor's starch-iodine test can show exactly where the sweat glands are most active: tincture of iodine is applied to the skin and allowed to air-dry, then the area is dusted with cornstarch. When sweat reaches the surface, the starch and iodine combine and the color changes dramatically from yellow to dark blue, making sweat production easy to see.
Cleansing. The treatment area is cleansed with an antiseptic to keep the chance of complications to a minimum.
Numbing. Numbing cream is applied and left for 45 minutes to reach its full effect, so the treatment is as painless as possible.
Injection. Using the finest-gauge needle, the botulinum toxin is injected just under the skin, evenly across the whole treatment area.
After the injection. You can return to your normal activities straight away.
Your doctor sets the number and timing of repeat treatments after an in-person assessment. Several treatments can be combined on the same day, as your doctor advises.
Underarms
Most patients receive about 40 small, evenly spaced injections in each underarm, approximately 8 mm apart. Each injection is given in one smooth motion to reduce trauma to the skin.
Hands and feet
The palms and soles are rich in nerve endings, so they are more sensitive, and treatment needs many small injections to cover the area. We take comfort seriously and use everything at our disposal to make the experience as painless as possible: numbing cream beforehand, ice compression to dull the sensation and, importantly, no rush. Taking our time is part of the comfort. Most patients find it far more tolerable than they expected.
After hand or foot treatment, ice packs are applied while you rest in the treatment room for 15 minutes, so we can confirm there is no immediate reaction.
What to expect
Sweating starts to decrease 5 to 7 days after treatment, and the result is obvious after 2 weeks. Many patients notice the effect even sooner. Odor and skin color improve at the same time as the sweating decreases, and so does quality of life.
The result can last up to a year after underarm treatment, and in most people it lasts around 9 months. For the hands, studies report reduced sweating lasting roughly 4 to 12 months per treatment (efficacy review, PMC 2024). Results vary between individuals, so we plan your maintenance around how your body responds. When your symptoms begin to return, which can be anywhere from 6 to 9 months, simply come back for a repeat treatment.
Before and aftercare
Before your treatment
Make sure you do not have a skin infection in the treatment area.
For underarm treatment, shave your underarms the night before.
After your treatment
A few spots of pinpoint bleeding may appear after the injections; they stop on their own.
Bruising is possible and also resolves on its own.
Keep the treated area clean and dry, and do not use deodorant for 24 hours.
Shower as normal.
Avoid dirty activities and heavy exercise for 24 hours.
Avoid saunas for 2 weeks.

The science
How botulinum toxin stops sweating
Research shows that botulinum toxin abolishes sweating by making the sweat glands less responsive to acetylcholine, the nerve signal that activates them (Botulinum toxin abolishes sweating via impaired sweat gland responsiveness to exogenous acetylcholine, British Journal of Dermatology / PMC).
Quality of life and satisfaction
Patients who have received injections report a significant reduction in excess sweat production and a better quality of life. They describe less occupational and emotional impairment, less time spent managing their sweating, and greater comfort in social and work situations. In placebo-controlled trials, patient satisfaction was much higher with botulinum toxin than with placebo (93% vs 30%). Patients also report higher satisfaction with botulinum toxin than with non-surgical alternatives such as topical agents.
Palmar hyperhidrosis
A 2024 literature review of botulinum toxin A for palmar hyperhidrosis reports reduced sweating lasting from about four to twelve months after treatment (The Efficacy of Botulinum Toxin A in Treating Palmar Hyperhidrosis: a Literature Review, PMC 2024).
Injection technique described in the literature
A published review of hyperhidrosis treatment with botulinum toxin (Doft MA, Hardy KL, Ascherman JA. Treatment of Hyperhidrosis With Botulinum Toxin. Aesthetic Surgery Journal 32(2): 238–244) describes the technique our approach is based on:
Underarms. Injections are given with a 26- or 30-gauge needle on a tuberculin syringe, just under the skin, with the needle at a 45-degree angle about 2 mm deep. About 40 evenly distributed sites are used in each underarm, approximately 8 mm apart. The total dose per underarm is 50 units, which was found to be the minimal dose needed to stop sweating completely in healthy volunteers. The 50 units are usually diluted in 2 mL of saline, and up to 3 mL in patients with larger underarms.
Palms. Sites are placed about 8 mm apart, with the needle at 45 degrees, bevel up, about 2 mm into the skin, depositing about 0.05 mL at each site. A total of 100 units per palm is diluted in 3 to 4 mL of saline and injected into each 1-cm square of the palm, with three sites in each finger.
Soles. The soles present the same difficulties as the palms: both are particularly sensitive and have thick skin, which makes topical treatments less effective. Because the sole is larger than the underarm or palm, 150 units diluted in 6 to 8 mL of saline are injected following a grid of sites.
Pain control. Because of the rich nerve supply of the palms and soles, pain from the numerous injections can deter patients, so topical lidocaine (numbing) cream is applied beforehand.
Observations. Curiously, sweating often decreases more in the non-dominant hand. The most commonly published side effect of hand treatment is muscle weakness, noticed mainly when pinching or gripping with full strength (maximal opposition of the thumb); it is usually temporary and lasts a few weeks.
